Cleansing Caltheris's Sanctuary in Rex Infernus is essential for the main easter egg quest, but it does require a lot of work to get right. What should be one step is a series of mini-steps, which is why this guide will go through each step you need to take to cleanse Caltheris's Sanctuary, just in case you're stuck on any of the parts leading up to the finale in Rex Infernus!
Once you have repaired the Nexus Forge with the Void Talon, you need to interact with the outer, middle, and inner cranks on the pillars around Pack-a-Punch until they align with Caltheris's Sanctuary and shoot a laser beam into her mouth. This is simplified with the Cleanse the Temples puzzle solver at rex-infernus.netlify. Input the rotation direction, where each crank is located, and make sure your target is set to Caltheris.
The Shimmering Thread spawns randomly from purple orbs, which are found in the sky around the map. Pull these orbs to the ground with the Void Claw, and then interact with them. There's a small chance that the Shimmering Thread will spawn, and when it does, you can place it in the brazier within Caltheris's Main Chamber.
You can now use an Astral Flame on the brazier, and you can get this purple flame by pulling a purple orb into Pack-a-Punch and cranking the wheels. Once the brazier is lit, a zombie will be infected with mist, and you can get the Titan Trap at Caltheris's Passage to kill this zombie, and if done correctly, Caltheris will say a quote.
Inside Caltheris's Main Chamber, you will find a stone tablet on the ground when facing east toward the brazier. Interact with the tablet and stand on it. While standing on the tablet, hold the aim button to ADS. After enough time has passed while ADSing, the tablet will reveal a dial that you can send into the air with a charged shot of the Warden's Blight.
You now need to shoot the dial so that the laser beam is concentrated at the northeast wall just to the right of the blue crystal, as shown above. Shoot the middle ring of the dial so that you can slowly move it into position, as the outer ring will cause it to go too far wide. Just make sure the placement of the laser is exactly where we've shown in the image above!
Kill zombies underneath the dial, and the laser will destroy the wall it's focused on, revealing a stone statue. You can shoot the stone statue to reveal four Shadow Souls, which you can kill with any weapon, but Light Mend and Warden's Blight are super effective. With these four Shadow Souls dead, destroy the stone statue to end the lockdown.
Take the Void Claw and pull a purple orb to the stairs at Caltheris's Passage, and then use the charged Warden's Blight shot to send the orb up into Caltheris's head, which will make it glow purple.
