Black Mushroom is a very rare consumable item in Diablo 4 that players may occasionally find inside Waking Nightmares, Season 15's time-limited activity that sends players back to some of the series' iconic dungeons and locations. Black Mushrooms were first introduced back in Diablo 3, where they were critical to crafting the Staff of Herding item. In Diablo 4, these don't seem to have any practical use, though they might still be tied to an easter egg that players are yet to discover.
Waking Nightmare dungeons appear all throughout Sanctuary during Season 15, and they'll take you back to a specific location from one of the older Diablo games, like the Den of Evil from Diablo 2 or Lester's Farm from Diablo: Hellfire. Inside these dungeons, there is a very small chance that you'll find a patch of dark mushrooms somewhere — interact with it, and you'll get a Black Mushroom.
You can replay any Waking Nightmare dungeon that you've visited before by using a Chronicle of Nightmares. These act much like Nightmare Dungeon Sigils — activating a Chronicle of Nightmares lets you teleport straight to a modified version of a specific Waking Nightmare. You'll earn more rewards, but at the same time, there will either be stronger enemies, or modifiers that will make your run a bit more complicated than usual.
If you don't find a Black Mushroom during your first Waking Nightmare dungeon run, then you'll have to do a different dungeon, or use a Chronicle of Nightmares to replay the one you just finished.
As of writing, the only thing Black Mushrooms do is make your character vomit when consumed. In Diablo 3, these were used as part of the crafting recipe for the Staff of Herding, an item that opens a portal to the Whimsyshire easter egg area. It's very likely that Black Mushrooms are simply easter eggs that have been included to represent the goofy side of Diablo 3, as there Wirt's Bell, Leoric's Shinbone, and Gibbering Gemstone don't seem to be in-game, nor are there any recipes for the Staff of Herding in Diablo 4's crafting vendors.
However, Liquid Rainbows do exist in Diablo 4, and instead of being a crafting material, they have been turned into consumables that directly open a portal to Whimsyshire, removing the need to craft the Staff of Herding. It's possible that Black Mushrooms can be combined at the Horadric Cube with other mysterious items added in Season 15, but as of this time, it's hard to confirm if this is true.
Whimsyshire is a magical place full of green grass, rainbows, clean water, and cute, cuddly animals that you can slaughter for loot. It's an easter egg in the same vein as the Secret Cow Level, and if you skipped Diablo 3, you can experience it in Diablo 4 by using a Liquid Rainbow bottle or by entering a portal that randomly opens somewhere in Sanctuary.
