Keira Metz is one of the characters Geralt can romance in The Witcher 3, but getting this outcome depends on a few specific dialogue choices during her quests. You will first need to make the right decision during the Wandering in the Dark quest, then continue helping Keira with her requests after lifting the curse on Fyke Isle. This guide covers the romance-related choices and the steps you need to follow to unlock her romance.
During Wandering in the Dark, Geralt and Keira will eventually reach a section where you must stay inside her magical shield while dealing with the Hounds of the Wild Hunt. Several rifts will appear throughout this section. Once the final rift is closed, Keira will fall onto Geralt, triggering a timed dialogue choice. You will have two options:
Quickly select the first option, Just don't faint on me, to increase Keira's romance level. This is an important choice if you want to pursue her romance, so make sure you select it before the timer runs out.
After lifting the curse on Fyke Isle during A Towerful of Mice quest, return to Keira's hut. She will have another request for Geralt, asking him to retrieve a package that has not arrived.
Travel to Hangman's Alley and use your Witcher Senses to locate the tracks left by the missing wagon. There are Drowners and Water Hags in the surrounding area, so deal with them if they become a problem.
Follow the tracks away from the road until you reach a broken-down wagon. Search beneath the wagon to find a small box containing Keira's Package. Pick it up and return to her hut.
After handing over the package, Geralt will inquire about the contents of the package to which Keira will ask him to stay a bit longer to find out. You can decline her request and leave if you want, returning later to continue the quest. However, if you want to pursue her romance right away, agree to stay.
Once you head outside, Keira will transform some white mice into horses and ask Geralt to play the role of Prince Charming in her fantasy. You can either agree to play along or refuse. Choose the first option to continue the romance path. Refusing will cause the quest to fail at this point.
