Mutations are a new mechanic introduced in Blood and Wine's expansion, and they're vital for making your build even stronger than it already is. Mutations are exclusive to Blood and Wine, and they require you to complete "Turn and Face the Strange", a side quest in Toussaint that you should obtain after "The Beast of Toussaint" main quest while you're exploring Beauclair.

You can spend Greater Mutagens and Ability Points to unlock Mutations from your Character menu (press Y/Triangle). While you can, and should, research all 12 Mutations, you can only equip one active Mutation at a time.
Unlocking Mutations, even if you don't use them, will allow you to unlock additional Ability Slots to equip more skills. You can have four more skills with the Mutations system active:
The skill you use in these bonus slots must be the same color as the active Mutation. However, if you have a "hybrid" Mutation (for instance: a Mutation that was unlocked via Green and Red Mutators), then you can use either a Green or Red skill!
Skills used in the middle column (bonus slots) are not tied to Mutagens, so you won't gain additional bonuses.
Alchemy: Each time you are injured in melee combat, the attacking opponent receives damage in the amount of 1.5% of damage dealt for every point of your Toxicity level (currently 1.5%, to a maximum of 150%).
