Ethan Winters' entire reason for visiting the Baker mansion in Resident Evil 7: Biohazard is because he received a message from his wife, Mia, who has been missing for three years. Ethan was convinced she was dead, so naturally he drops everything and heads to Louisiana to find her.
Well, he finds her all right. Although all the Bakers are terrifying in their own way, Ethan doesn't meet any of them off the bat. Instead, he finds Mia, but she's not quite herself. The moment you see her crawling up a dark stairwell, it's pretty clear that something has gotten in her, and it wants to kill Ethan. While you do have some control during this scene, from seeing Mia in the stairwell to when Ethan picks up a hatchet to fight back, it's pretty much just one harrowing cutscene, and is easily the scariest moment in the scariest Resident Evil game.
Before you even start playing Silent Hill f, the game hits you with a screen detailing some trigger warnings. It mentions some thematic stuff like child abuse and gender discrimination, but also includes torture and graphic violence. But this is a Silent Hill game, right? That stuff is par for the course. In this case, you should absolutely take those warnings seriously.
About mid-way through the game, Hinako is exploring the Otherworld (a Silent Hill staple) where she's been betrothed to a strange man named Fox Mask. To prove her devotion, Hinako must undergo a "ritual," and it's pretty ugly. First, she cuts off her own arm at the shoulder and has it replaced with a fox arm. Then, she receives a brand the size of a basketball on her back. Finally, she cuts off her own face and replaces it with a fox mask. It's horrifying, and Silent Hill f really takes its time with the whole scene. It's a really hard watch, and easily the most unforgettable moment in the entire game.
Bloodborne is absolutely loaded with horrific creatures for you to fight, both standard enemies and bosses. Giant, beastly abominations are the game's bread and butter, but things hit a completely new level with Ludwig the Accursed in The Old Hunters DLC.
