Blood Simple is the next quest in The Witcher 3: Blood and Wine if you decide to find Orianna instead of seeking out Damien in The Night of Long Fangs. Geralt and Orianna head to her orphanage, only to find the children slain, with just one little boy as a survivor. Geralt must find the vampiric threat that did this and decide if he truly wants to go through with finding the Unseen Ending, as this will have immense consequences for the story going forward!
If you started "Blood Simple" but want to go back to "The Night of Long Fangs" to change your mind, then don't reload a save! You're given that choice at the end of the mission.
When you're tasked with searching the orphanage with your Witcher Senses, head outside and go down the stairs. At the left pillar, you'll see green goo that you can examine. This will give you a trail to follow, which takes you to a fence with a corpse covered in the green goo.
Examine the corpse, and Geralt will comment that this is the work of an Alpha Garkain. You can now read the entry in your Bestiary under the Vampiric section to find information on the Alpha Garkain, which rewards 100 XP.
Now, examine the wagon tracks and the paw prints, where you'll be tasked with following the wagon tracks off-road to the north on horseback. Eventually, you will find the wagon on the shoreline, but if you use your Witcher Senses and face northwest, you'll be able to find the Alpha Garkain's hideout under the bridge.
The Alpha Garkain is a fairly easy fight, as it's pretty slow and vulnerable to Moon Dust, Samum, Vampire Oil, and Black Blood. Once it's dead, you can loot it for the Garkain Trophy, which provides +5% bonus XP from monsters.
There's also the Letter from Orianna on the corpse it was feeding on, which Geralt will comment about, but it offers nothing except additional lore.
With the Alpha Garkain dead, you can return to the orphanage to tell Orianna the good news. However, when you arrive, you'll find Orianna feeding on the young boy's blood, and she exclaims that she feeds on children, but never enough to kill them, in exchange for shelter and food. Geralt can respond in two ways:
