Geralt reunites with Regis, a higher vampire and an old friend, in The Witcher 3: Blood and Wine's main quest, La Cage au Fou. In this main quest, Geralt must get the saliva from a Wight, and this opens up an optional moral choice about whether you should attack the monster or try to lift her curse. As you arrive at the Mere-Lachaiselongue Cemetery, you'll knock on the door only to receive no reply from Regis. Instead, head left to find an open tomb that you can vault into to drop into the tunnels below. Inside, you'll have to fight Kikimores, so make sure you use Igni to burn all the eggs in the room for an extra 100 XP.
Deeper into the tunnels, you can take a right to the main objective. However, take a detour to the left and you'll find an illusion wall you can interact with thanks to the Eye of Nehaleni. This will reveal a stone tomb that has the Casus Foederis sword for you to collect. Regardless, return to the path and use Aard on a stone wall to find Regis.
Regis will ask Geralt if he wants a drink, and while your response has no consequence on the story and your friendship with Regis, your choice will change the amount of time you spend speaking with him:
I decided to drink with Regis, as he is an old friend of Geralt's, so it's nice to see them talk to each other. No dialogue options matter here, but when Regis asks Geralt if he'd choose to be a witcher in a new life, I said "Yeah. I'd be a witcher," as it's clear Geralt loves this life.
After the cutscene, Regis will inform Geralt that he needs the saliva of a Spotted Wight. For 100 XP, you can enter the Bestiary and read the Spotted Wight entry in the Necrophage section. Now, just head to the Trastamara Estate Ruins to locate the Wight's Lair, but be mindful of the Barghest that lurks in the fields!
Inside the lair, you'll need to activate your Witcher Senses and examine as many red items as you can. Focus on the writings on the walls and the following documents:
Head into the cellar, and you'll find a cauldron that you can interact with. This will prompt Geralt to hide, so turn around and interact with the closet.
