The main story for The Witcher 3: Blood and Wine has come to an end, but there's still an epilogue to get through to see if you managed to give Geralt a happily ever after. The epilogue to Blood and Wine, Be It Ever So Humble, takes Geralt back to his vineyard at Corvo Bianco, where he will meet an uninvited (and potentially surprising) guest. Be It Ever So Humble will only be added to your main quests tab if you have completed The Witcher 3: Wild Hunt's main story with "Something Ends, Something Begins," and Blood and Wine's main story with "Pomp and Strange Circumstance" or "Burlap is the New Stripe".
Once you have the new quest available, head to Corvo Bianco, where you will find your majordomo, Barnabas-Basil Foulty, waiting for you outside. Barnabas will inform you that a guest has arrived, but who it is, depends on the decisions you made in the main story.
Yennefer retires with Geralt at Corvo Bianco, bringing along her books and items so that she can move in and enjoy their happy ending.
Triss tells Geralt about Kovir, and that she's only visiting before she returns north.
Geralt reunites with Ciri. If she's a witcher, she will express her love for her newfound freedom (out of Geralt's shadow), but if she's empress, she'll express how she might bail from that destiny.
Don't romance Triss or Yen (or romance both).
Ciri became a witcher or the empress in the main story.
