Old Tristram returns as an easter egg in Diablo 4 Season 15, but while there might not seem like much going on in the town where it all started, players can actually fight Uber versions of the three Prime Evils here once they get the right item. Wirt's Coin, another secret item in Diablo 4, can be crafted using the Horadric Cube by combining the Splinters of Terror, Hate, and Destruction. These are extremely rare, but if you want to get them all and fight the three brothers all at once, here's what you need to do.
You can get to Old Tristram by finding all of Deckard Cain's journals hidden across Sanctuary. There are 10 of them in total, and they'll appear on your minimap as a small, colored blip once you get close enough. We have a detailed guide on where you can find all of Deckard's Journals in Diablo 4, but here's a quick rundown on each of their locations:
After you get all of Deckard's journals, a new seasonal quest will pop up. Go back to the Imperial LIbrary and speak with Deckard Cain. This will open a new portal nearby. Enter it to reach Old Tristram.
Once you're done opening the Old Tristram portal, you can open another portal leading to Uber Tristram by using Wirt's Lucky Coins on the fountain in Old Tristram.
To craft Wirt's Lucky Coins, you need to combine the Splinters of Destruction, Hate, and Terror that drop from the Echoes of Baal, Mephitso, and Diablo on Torment 10 in their respective Waking Nightmare dungeons on Sanctuary. All three Splinters can drop from any of the brothers, so you can farm just one of them (I'm not sure if this is intentional, though, and it might be changed later in the season).
After you get Wirt's Lucky Coins, go back to Old Tristram and interact with the fountain there. This will open a portal that leads to the Echo of Tristram, where you'll fight all three brothers at once.
The Uber Prime Evils have extremely high health. You need to be strong enough to comfortably farm Torment 12 to even have a chance at winning this fight, and even then, it's going to be tough. Beating the Prime Evils will reward you with the Hellfire Torch, a Mythic Unique Charm that gives a massive boost to your stats.
