Together Forever is a secret achievement in Blood of Dawnwalker that players can only get during the Home Sweet Home quest. To get this achievement, you need to complete the time loop in the Home Sweet Home quest at least five times, which is easier said than done. You won't get access to your normal gear, and your health will stay the same even after you repeat the cycle. Thankfully, you can still use your abilities in Blood of Dawnwalker, and if you're having trouble getting this achievement, here are some few tips you can try to make the grind less tedious.
First, you'll want to have the Soul Reaping ability unlocked. This isn't exactly necessary, but it'll let you keep your health up throughout all the combat encounters you'll be doing in human form. You'll lose access to your inventory while doing the Home Sweet Home quest in Blood of Dawnwalker, which means you won't be able to use any of your healing items.
Next, you'll need to start the Home Sweet Home quest by going to the Silts Tower, which is at the far southwestern edge of Vale Sangora. Go there at night, then enter through the door on the ground floor. Fight your way through the soldiers and the witch miniboss, then enter the top of the tower. After going through the door, you'll find yourself back at Coen's house in Laslea, with everything seemingly back to normal.
After the scene at family's home, your goal is to save Coen's family by beating some monsters and the winged boss that destroyed your house. Once you beat them all, go to the church, watch or skip the cutscene, then do everything all over again.
Getting this achievement unlocks a secret ending.
Fighting all the enemies repeatedly is a matter of attrition. You won't get many healing items throughout your stay in the time loop, so if you get hit, all that lost health is going to be lost for a while.

If you don't have Soul Reaping, keep an eye out for lootable barrels and other containers in the village. They tend to have items for restoring health and stamina, and they refresh with every loop.
