Super Saiyan 5 is perhaps the longest-lasting urban legend in Dragon Ball history, but despite its reputation, the form actually does have some legitimate basis in the actual franchise. The concept of a “Super Saiyan 5” has been around since the end of Dragon Ball GT, with Vegeta (or Bulma depending on the dub) acknowledging the transformation while working on getting him SSJ4.
Not only that, the Dragon Ball Super manga’s Toyotarou has actually designed his own version of Super Saiyan 5 in Akira Toriyama’s style. Super Saiyan 5 as most people know it mainly exists in fan works, but Dragon Ball Super technically has its own version of SSJ5 – and it’s absurdly powerful. With Dragon Ball canonizing Super Saiyan 4 in DAIMA, who's to say Super Saiyan 5 isn't on the horizon? Goku’s going to need a new form if he's going to beat Black Frieza, after all.
Many Dragon Ball fans are familiar with the image of Super Saiyan 5 Goku from the fake Dragon Ball AF hoax. What most fans don't know is that this artwork is not of Goku at all, but of an original character by a Spanish DBZ fan. The character is actually Spirit Killer Tablos, who seemingly has no connection to Goku.
Considering Tablos is clearly modeled after Super Saiyan 3 Goku, however, it's easy to see how fans in the late 90s and early 2000s made the connection. Spirit Killer Tablos would go to serve as a major inspiration for Toyotarou, who would repurpose the design into an official Super Saiyan 5 Goku form in his Dragon Ball AF fan comic.
Super Saiyan 5 doesn't actually appear in Dragon Ball GT, but it is mentioned. In the Japanese version of DBGT Episode 55, Vegeta comments that he'll unlock Goku's "Super Saiyan 4 or 5, or whatever." The English dub for the episode instead has Bulma mention that she can help Vegeta trigger Super Saiyan 5. In both canons, the idea of SSJ5 is merely alluded to.
As far as Dragon Ball GT is concerned, Super Saiyan 4 is the peak of Goku and Vegeta's power. That said, Gogeta's Super Saiyan 4 could really be Super Saiyan 5. The red hair is vastly different than Goku and Vegeta's natural hair cycle, and Gogeta's fur & aura have more of a shine to them. In a post-Battle of Gods world, GT Gogeta's design is strangely reminiscent of Super Saiyan God. It's worth noting that Gogeta is never actually called a Super Saiyan 4 in the episode itself, even though the title does make reference to SSJ4.
Before he became Akira Toriyama's official successor and started working on multiple different Dragon Ball manga, Toyotarou was a fan artist going by the name of Toyble. It was through his work on his own version of Dragon Ball AF that Toyotarou caught the attention of Toriyama, eventually being forced to end the doujin in order to start writing Dragon Ball Heroes: Victory Mission. Toyotarou's Dragon Ball AF ends on a cliffhanger, but not before Goku unlocks Super Saiyan 5.
