After Simon reaches the clinic of St. Amelia in Silent Hill: Townfall, he'll find three scales in Zoe's office with a note next to them. The note is long and won't seem like much of a clue at first, but the three lines at the bottom are actually hints about what needs to be placed on the scales.
Needless to say, you won't be able to progress beyond this point without finding the missing items, which, according to the note, represent listening, mistakes, and Zoe's job. You'll already have the item for the last scale, which is Zoe's badge that you used to get inside the clinic, but for the other two items, you'll need to go to two other locations in St. Amelia, one of which is the pharmacy.
You should have picked up a scalpel by this point, which will give you access to the pharmacy. Inside, you'll find empty shelves, a desk, an employee area, and a bathroom. At the back of the pharmacy, there will be one obviously identified locker with a 4-digit combination lock.
On the desk opposite the locker, you'll have your first interaction with a device you'll use with your CRTV to give you a new signal. The game will guide you since this is the first time, but essentially you'll want to move and tilt your controller until the feed lines up on the screen. When it does, it will sort of snap together.
At that point, you need to hold the controller in that position. If you don't, you'll lose the feed. While holding the controller still, use the up and down buttons on the D-pad to cycle through red, green, and blue, then use the left and right buttons to line up each color bar. After you've done all three, you'll be able to store the signal.
When the full footage comes through, you'll see flashes of various things inside the pharmacy, which will turn out to be some of the posters on the wall. You won't get much more out of the footage than this, but it clearly indicates that the posters have something to do with the locker, which, as you'll know from the voicemail system on the phone, contains a messed-up dosage.
There are four main posters in the pharmacy, including one in the bathroom, one on the cork board, one by the corner leading to the back area, and one near the front of the store. Each one will have something on it to indicate a number or something you need to count. For each poster, you'll need to note the following:
You need to take note of all these numbers, and they need to be in the order they're shown in the video: Hand Wash, Safe Medicine Steps, Blood Pressure, Antibiotics. As long as you counted correctly, the numbers you should have are 6539.
