If you're wondering if you should force-feed Esme or not in The Blood of Dawnwalker, then you should know that there are no consequences for whether you try to force-feed your mother or not. Although this is one of the first choices you will face in The Blood of Dawnwalker, and you might expect some consequences to explain the game mechanics, it's actually one of the many insignificant choices that has little to no consequences in the game.
The short answer is that it doesn't matter. Whether you choose to feed Esme by force while Pieter is restraining her, or refuse to feed Esme by force, it doesn't change anything in the story. The only way you can improve Coen's mother's condition is by correctly brewing the medicine from Anca.
If you choose to try and feed Esme, she will still refuse, and you will fail. If you refuse Pieter's order and don't force Esme to eat, Pieter will be frustrated all the same, but he doesn't do anything about it. Therefore, you can choose whichever option resonates with you better.

The reason Esme needs to eat in The Blood of Dawnwalker is that during the Mass, Brencis and his boyar will drain blood from every participant. If Esme refuses to eat, she will be too weak, and her blood will be too thin, unacceptable to Brencis. Later on, if you brewed the medicine from Anca incorrectly, your mother's condition won't improve, and she will remain weak during the Mass. This may lead to an unfortunate turn of events, since Brencis will be angered by Esme's condition.
However, if you manage to make the correct medicine brew, Esme will be slightly better. Although she's still weak and Brencis will comment about "culling the herd," he won't actually do anything, and your mother won't face any consequences during the Mass.
Unfortunately, there's no way for Esme to skip the Mass. For example, I tried going to the church and speaking to Father Florin to let Coen's mother skip the Mass, but he won't allow it. I even tried attacking Florin, but alas, he's unkillable in the Prologue. I also tried asking Zdislav, the village Elder, but he also couldn't do anything.
