Once you've opened the Organ Transplant box at Zoe's house and retrieved her badge, it will be time for Simon to make his way towards Silent Hill: Townfall's clinic. Zoe's bag will open the previously locked door, allowing entry, where you'll be free to explore behind the safety of a locked front door.
At the back of the clinic, you'll find Zoe's office, with a ton of boxes, a computer, and three precariously placed scales. Next to the scales will be a note, which, yes, indicates this is another puzzle you'll need to solve to progress. The scales are actually a larger puzzle with several smaller puzzles required to find the two other items you're missing, but at least you already have one item, the badge.
The first step towards solving this puzzle is to gather as much information as you can. The note next to the scales is the primary document that indicates what to place on each scale. The upper half is more for story purposes, but the lines you need to pay attention to are underlined and read as follows:
There's also a line underlined at the bottom, but it just tells you to show you understand by placing the correct items where they go.
Based on the information in the note, you need something that represents listening, mistakes, and Zoe's job. The last item on this list, thankfully, is already in your inventory. For the last scale, you'll need to place Zoe's badge that you found in her house and used to get inside the clinic.
As for the other two items, you'll need to head to other locations in St. Amelia to retrieve what's needed. If you haven't already, check the computer in the room to view Zoe's emails. There will be one from Jessica indicating that she found Zoe's medical kit and she's worried about her. It won't explicitly hint at the listening item, but the address indicates you need to head here.
Make sure you also grab the Scalpel in the medical kit on the same desk as Zoe's computer. You'll need it to reach the other areas where the other two items are.
After you read through the emails, head back up to the front and use the code on the sticky note at the computer to open the door there. Inside, you'll find a phone, where you can check the message mentioned in one of the emails. Listen to the message, and you'll find out that a messed-up prescription is waiting at the pharmacy. Naturally, that is the item you'll need for the mistake scale.
