If you choose to speak to Katja, you must bring up Nora, ask about Andrei, then offer her 400 Denarii to help with her problem. The easiest route is to wait until night and climb, which lets you skip a lot of the conversations and risks. Going through the butcher's door will cause Nora to be held hostage, potentially leading to her death. You don't have to save Nora if you simply climb the balcony at night.
