You can find the Broken Stethoscope Key Item in Silent Hill: Townfall by getting into Jess's apartment and examining the bag on the bed in her room. In its current state, the stethoscope doesn't work properly, but that actually doesn't matter. You don't need to find a way to fix the item — you'll need it for a certain puzzle in Townfall.
The broken stethoscope is one of three items you need for the scales puzzle back in Zoe's office at the clinic. By this point, you've probably visited this location already. The room has three scales, and you need to place three items on top of each one in a specific order.
Once you have all three of the key items in your possession, you need to head back to the scales and place the items in this order: Stethoscope, Error Note, and then the Lanyard. Completing this puzzle will end Part 1 of Silent Hill: Townfall, and then you'll start Part 2 with a "new perspective."
