The Witcher 3's first expansion, Hearts of Stone, begins with "Evil's Soft First Touches". Before starting "Evil's Soft First Touches", you should complete the main story and be at least Level 32.
There's a notice board at the Seven Cats Inn in Velen, where you can claim the Evil's Soft First Touches contract. After it's been read, you can travel to Brunwich to make your way to the Garin Estate to meet with Olgierd von Everec, but his band of Wild Ones is rather teasing to Geralt, presenting you with two dialogue options:
Regardless of your choice, you will speak to Olgierd upstairs, and you can comment on the statue or not; it doesn't matter. When you both go downstairs, make sure to haggle with Olgierd, as this will increase your payment to 450 Crowns.
When you leave the manor, you can agree to a job from a woman that begins the "Rose on a Red Field" side quest, which can be done later.
Head to the Oxenfurt market, and you will be stopped by two guards. They are trying to extort you for 50 Crowns, but you can refuse to pay without consequence, so make sure you save your money by telling them no! Afterward, head to the well and go down the ladder to enter the sewers.
Inside, you'll have to kill Drowners and meet with Shani. You'll find a locked door, but to the right is a wall you can break with the Aard sign. Continue on the path, and you can either jump down the hole to the left or explore the area for miscellaneous items.
Entering the door shown in the image will start a cutscene that puts you directly into the boss fight with the Toad "Prince", so make sure you're ready for a fight by bringing Golden Oriole to turn poison damage into healing and Cursed Oil for extra damage.
