You can upgrade to The Witcher 3: Remastered as long as you have purchased The Witcher 3 on the same platform you're trying to play it on. This includes:
Simply head to the digital storefront of your chosen platform and find "The Witcher 3"; you should find the digital version of "The Witcher 3: Wild Hunt - Remastered" as a free upgrade to download. This is available for free to everyone who owns the base game or the Complete Edition, including those who have a disc copy!
The Witcher 3: Remastered comes with Hearts of Stone and Blood and Wine as part of the download, meaning that just by installing The Witcher 3: Remastered, you will already have the two expansions by default!
Hearts of Stone and Blood and Wine are included in the Base Game for free, so if they're not installed by default, just check the digital storefront (PS4 and Xbox One), and you should be able to download them entirely for free.
