Simply put, it doesn't matter whether you stay silent or speak up during the Mass in The Blood of Dawnwalker. The story's path is set, and you'll soon see what kind of ruler Brencis is for Vale Sangora. Thankfully, while your choice during the Mass does not affect the story, the choices you make as a Dawnwalker can indeed shape the fate of Vale Sangora.
After everything you do in the Prologue of The Blood of Dawnwalker, the Mass will inevitably come. As an adult, Coen is forced to attend the disturbing worship of Brencis.

You have to listen to Father Florin's speech, which is no less disturbing than the whole ritual, and you'll soon have a timed-choice to make:
If you choose to say "This is sick," Pieter will scold Coen, telling him not to attract attention. If you stay silent, nothing will happen. Afterward, you can choose to repeat his words, "May it forever endure." Or [remain silent]. Again, neither choice has any impact on the story, so you're free to choose whichever.
Once a week, adult villagers over the age of 18 must attend a Mass. Since only adults should attend, Coen's siblings, who are Yanna, Mirto, and Lunka, can all stay home and not watch the horrific rituals. Those who do attend, though, have to perform a blood exchange with the Knyaz and his Boyars.
The people will drink Brencis's blood, which has the power to heal wounds and physical sickness (mental sickness, such as Esme's problem, won't be solved by Vrakhir blood), and then they must provide their own blood for the Vrakhir to consume. This is because the vampires must drink blood to survive, and whilst animal blood may suffice for a while, they'd crave human blood once in a while. So it's a mutual exchange where one gains health and the other satisfies their cravings humanely.
But, once you understand the true purpose of Brencis's ruling over Vale Sangora, you'll soon know that his goal is far from pure. While it's true that Brencis annihilates the plague from the valley and dethrones a tyrant, he himself is no less a tyrant than the previous Knyaz, although he tries not to present himself as such.
